Job Description: REGISTERED NURSE / Clinical Field Staff

QUALIFICATIONS

Graduate of an accredited nursing school. Must possess a current license to practice nursing in the state of practice. Minimum of one (1) year of experience in general nursing practice. Should have supervisory and good communication skills. Ability to work well with others, function well under pressure, and function independently. Must comply with the standards outlined in the Nurse Practice Act.

REPORT TO: DIRECTOR OF PATIENT CARE SERVICES / ADMINISTRATOR

JOB DUTIES (INCLUDE, BUT ARE NOT LIMITED TO THE FOLLOWING)

  • Provides skilled nursing services in accordance with the orders of a physician and under a plan  of care in a patient's home. Has the responsibility for the observation, assessment, nursing diagnosis, counsel, care and health teaching of the ill, injured, or confirmed, and the maintenance of health and prevention of illness of others.
  • Records the health needs of the patient.
  • Performs and completes start-of-care, recertifications, resumption-of-care, and discharge Comprehensive OASIS Assessment and associated paperwork in accordance to agency policy.
  • Initiates Plan of Care revises as necessary.
  • Evaluates the effects of the care given and regularly reevaluates the patient's nursing needs.
  • Provides those services requiring substantial specialized nursing skill based on the physician's orders and the nursing diagnosis. Examples of such services include, but are not limited to; physical assessment, patient and family teaching, urinary catheter insertion and care, venipuncture and administration of IV fluids or medications, dressing changes, sterile and nonsterile.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Counsels the patient and his/her family in meeting nursing and related needs.
  • Acts as the coordinator of the health care team in order to maintain the proper linkages with a continuum of care.
  • Coordinates the care of all patients in his/her case load.
  • Makes referrals to other agencies or services as needed.
  • Provides summary reports to the patient's physician about patient under care when the patient's condition changes or there are deviations from the plan of care, or at least every 60 days.
  • Prepares clinical and progress notes on the relevant and specialized nursing services he/she provides and maintain a clinical record for each patient receiving care.
  • Submission of documentation per agency policy.
  • Participates in Clinical Record Reviews.
  • Participates in in-service programs and staff meetings.
  • Directs the activities of the licensed practical nurse.
  • Makes the home health aide assignment, prepares written instructions for the aide and supervises the aide in the home.
  • Complies with Homecare Agency Compliance Plan.
  • Complies with Homecare Agency's Infection Control Policies.
  • Complies with Homecare Agency's HIPAA Policies.

  • Maintains weekend, holiday and vacation schedules. Assist Clinical Coordinators in completing weekend and holiday assignments and arranges vacation coverage for field staff.
  • Data Entry of field staff assignments.
  • Participates in quality improvement activities.
  • Assist with orientation and training of other staff members.

KNOWLEDGE, SKILLS AND ABILITIES

Prefer clinical certification such as LPN, PTA, or equivalent and a minimum of one year's clinical experience. Home care experience strongly preferred.

Excellent communication and organizational skills. Knowledge of office machines required. Employee is required to sit, stand and walk frequently; and to lift up to 50 pounds occasionally.

Job requires occasional bending, kneeling, reaching forward and overhead, and lifting floor to

waist and waist to overhead. Position involves frequent CRT terminal work, requiring continuous sitting and use of both hands for grasping, pushing/pulling and fine manipulation;driving requires use of at least one foot.

JOB DESCRIPTION SUMMARY

The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs.  The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed.

The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care.

QUALIFICATIONS

  • Meets the training requirements in accordance  with State and Federal laws.
  • At least 18 years of age.
  • Ability to read and follow written instructions and document care given.
  • Self-directing with the ability to work with little direct supervision.
  • Empathy for the needs of the ill, injured, frail and the impaired.
  • Possess and maintains current CPR certification.
  • Demonstrates tact, patience and good personal hygiene.
  • Licensed driver with automobile that is insured in accordance  with Organization requirements and is in good working order.

  • Assisting in ambulation and exercise according  to the plan of care.
  • Performing range of motion and other simple procedures  as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
  • Assisting  patient in the self-administration of medication.
  • Doing patient's laundry, as appropriate.
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
  • Reporting on patient's condition and significant changes to the assigned nurse.
  • Adhering to the Organization's documentation  and care procedures and standards of personal and professional conduct.

PHYSICIAN AND COMMUNITY LIAISON

Job Description:   The Physician and Community Liaison is primarily responsible for developing and implementing a marketing and sales plan to maintain and grow referrals from providers and/or physician practices to Linx Healthcare / Linx Home Health Care. Executes initiatives related to increasing loyalty and preference to Linx Healthcare / Linx Home Health Care affiliates among internal and external provider referral sources.  

QUALIFICATIONS

Bachelor’s degree in Communications, Business Administration, or Healthcare required.  Recent and direct experience working in a communications position within a sales or marketing work 

environment may be substituted in lieu of education and/or experience requirements. Experience: Minimum of five (5) years’ experience working with physicians/providers, medical practice office staff, or other medical specialties required. 


 Knowledge, Skills, and Ability Requirements 


  • Possesses outstanding interpersonal skills, including excellent written and verbal skills.
  • Demonstrates ability to use persuasive communication, leadership, and relationship-building.

  • Works collaboratively with service line leaders, Site Managers, and others to gather feedback/research and executes communication strategies to persuade provider referral sources to send patients to Linx Healthcare.
  • Develops and implements specific outreach, relationship building, and marketing plans to meet goals, in collaboration with Marketing & Public Relations.
  • Develops strong business relationships with local community organizations and businesses to increase their advocacy of Linx Healthcare / Linx Home Health Care.
  • Works collaboratively with other system functions that interact with physician offices, including hospital medical affairs, referral, provider recruitment, provider resources, etc.
  • Establishes and maintains ongoing communications with referring providers and/or their office staff, including routine or follow-up visits, emails, and phone calls.


Communication Planning


  • Using research-based methods, implements strategies to build awareness and trust between Linx Healthcare / Linx Home Health Care and applicable referral sources.
  • Identifies referral sources (target audiences) for each Linx Healthcare / Linx Home Health Care, service, or practice.
  • Identifies tactics to promote provider interaction and engagement including new provider orientation, site visits, staff meetings, social events, grand rounds, referral dinners, CMEs, and other communication methods.
  • Develops meaningful relationships with key contacts and opinion leaders including Site Managers, Nurse Managers, office staff, and others.


Data Collection & Analysis


  • Gathers and interprets the data necessary to determine priority areas for growing referrals to Linx Healthcare / Linx Home Health Care.
  • Oversees efforts to ensure Linx Healthcare / Linx Home Health Care is positioned competitively to protect and grow market share. This includes routine analysis of the strengths and services of competitors.
  • Develops and implements an ongoing method of profiling and monitoring market areas to identify. demographic, competitor, and changes in healthcare services.
  • Maintains reports related to volume trends for Linx Healthcare practices and hospital services.
  • Uses Press Ganey or other customer survey scores from practices and hospital services to identify opportunities to correct problems that have a negative impact on referrals.
  • Measures and evaluates liaison activities including producing and sharing regular summaries of patient visits and their outcomes as well as their impact on referral trends.


Communications Coaching


  • Coaches providers on effective communication and presentation skills for speaking engagements.
  • Coaches Site Managers and service line leaders on effective communication skills for outreach to referral sources.


Leadership Program


  • Collaborates as needed with Linx Healthcare / Linx Home Health Care personnel to develop a Provider Leadership Program.
  • Provides coaching on communication and outreach skills to new Managers during orientation.


Advisor & Advocate


  • Provides insight to Senior Management related to emerging business opportunities.
  • Advocates for initiatives aimed at improving processes related to referrals (i.e. increase convenience for referring providers to contact a practice).
